Welcome to the Department’s site for requesting records and for requesting public records defined by s. 119.011(1), F.S. The Department of Children & Families personnel are committed to fulfilling your request as quickly and efficiently as is reasonably possible.
Looking for Florida Records
Please use the attached newly updated request form and submit your request for an abuse history search via the email address listed at the bottom of the form. This form includes a space to inform us the reason for request (i.e. Relicensing or Annual.)
- You must list your facility OCA number on the form.
- All applicant demographics must be provided on the request form and match the demographics in the ClearingHouse.
- You must list your license expiration date for relicensing purposes.
- All applicant demographics must be provided on the request form along with the applicants expected position upon employment, new and veteran employees.
- Be sure to complete the "Reason for Request" section. The mentioned section is required for processing.
- Annual Screening is for facilities who choose to renew their applicants yearly based on the last abuse history date.
- Relicensure/Re-Screen is for facilities who choose to renew their applicants abuse history on an annual basis per their license expiration date and align their new hires with their relicensing date.
- You must list your facility License number on the bottom of the form. If you do not know what your license number is, please contact your licensing point of contact for the information.
- When submitting your requests, make sure you submit them one attachment per email or fax.
- We require this information to conduct a thorough search and without it our processing time is slowed down considerably.
- We do accept eSignatures on submitted request forms
- Please allow 5 – 10 business days for processing

Child Abuse History Record Request for Child Care Personnel Employment
Please see and fill out the attached form per your request for child placement purposes.
- All information must be legible
- The form is a fillable form. Please type the information while filling out the form
- The bottom of the form must be filled out by the requesting agency. The requesting agency is the adoption agency, foster licensing agency, child placing agency, or your state’s child welfare agency.
- All submissions for employment in a child placing agency must have the bottom filled out and submitted by your licensing rep and/or your state’s child welfare agency.
- When submitting your requests, make sure you submit them one applicant per request form, one request form per attachment, and one request attachment per email or fax.
- There is not a fee for an abuse history search from the Florida Department of Children and Families.
- Please complete the form, have it signed by both your agencies representative and the applicant, then return it to us for processing.
- We do accept electronic signatures and forms with typed information.
- We require this information to conduct a thorough search and without it our processing time is slowed down considerably.
- We strive for a 5 – 10 business day turn-around time.
- If 10 days lapse after you have submitted your completed form, please reach out to us for a status update.

- All information must be legible.
- The form is fillable, please type the information while filling out the form.
- All available applicant demographics must be provided on the request form. The Date of Birth is required.
- You must select the reason for request.
- The bottom of the form must be filled out by the requesting agency. The requesting agency is either the individual investigating abuse/neglect reports or ongoing cases with your state’s child welfare agency.
- The agency phone number must be able to reach the requestor for any questions we may have.
- When submitting your requests, make sure you submit them one applicant per request form, one request form per attachment, and one request attachment per email.
- We require this information to conduct a thorough search and without it our processing time is slowed down considerably.
- We do accept eSignatures on submitted request forms
- Please allow 5 – 10 business days for processing, it can take as many as 14 business days.
